Administration
• Fluoroquinolone Antibiotic Levofloxacin (Generic Levaquin) can be taken with a full glass of water, usually taken with a meal or within 30 minutes of a meal.

• For Nosocomial Pneumonia
Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 7 to 14days

• For Pneumonia
Levofloxacin 500mg once a day for 7 to 14days
Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 5days

• For Chlamydia Infections
Levofloxacin 500 mg once a day for 7 days

• For Sinusitis
Acute bacterial sinusitis: Levofloxacin 500 mg every 24 hours for 10 to 14 days
Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 5days

• For Bronchitis
Acute bacterial exacerbation of chronic bronchitis: Levofloxacin 500 mg once a day for 7 days

• Skin or Soft Tissue Infection
Uncomplicated: Levofloxacin 500 mg once a day for 7 to 10 days
Complicated: Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 7 to 14 days

• For Prostatitis
Chronic bacterial prostatitis: Levofloxacin 500 mg every 24 hours for 28 days

• For Urinary Tract Infection
Complicated: Due to Enterococcus faecalis, Enterobacter cloacae, E coli, K pneumoniae, Proteus mirabilis, or Pseudomonas aeruginosa:
Levofloxacin 250 mg once a day for 10 days as directed

Due to E coli, K pneumoniae, or P mirabilis:

Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 5 days as directed

Uncomplicated: Levofloxacin 250 mg once a day for 3 days as directed

• For Pyelonephritis
Acute: Due to Escherichia coli:
Levofloxacin 250 mg once a day for 10 days
Due to E coli (including cases with concurrent bacteremia):
Levofloxacin 250mg x 1 and Levofloxacin 500mg x 1 as a single dose every 24hours for 5 days

• For Cystitis
Levofloxacin 250 mg once a day for 3 days

• For Anthrax Prophylaxis
Prophylaxis post exposure to inhalational Bacillus anthracis: Levofloxacin 500 mg once a day for 60 days after exposure

• For Inhalation Bacillus anthracis
Mass casualty treatment: Levofloxacin 500 mg once a day for 60 days

• For Tuberculosis -- Active
Levofloxacin 500 to Levofloxacin 500mg x 2 as a single dose every 24 hours per day, for 18 to 24 months, or for 12 to 18 months after culture results are negative

• For Nongonococcal Urethritis
CDC recommendations: Levofloxacin 500 mg once a day for 7 days
Levofloxacin therapy is recommended as an alternative regimen. Single-dose Azithromycin (Generic Zithromax) is a preferred regimen.

• For Pelvic Inflammatory Disease
Mild to moderately severe acute PID: Levofloxacin 500 mg orally once a day for 14 days as directed
